Description of Work
I. Direct patient care- Hospital responsibilities
a.Perform admission work-ups and H&P; for Invasive Cardiology patients undergoing procedures requiring admission including percutaneous coronary intervention, pacemaker or ICD implantation, complex and/or uncomplicated ablation, antiarrhythmic loading, device generator exchange, lead revision.
b. Participate in daily rounds, perform physical examination and identify abnormal findings.
c.Perform medication reconciliation and enter admission/discharge orders in Epic, order appropriate pharmacologic agents and/or diagnostic tests.
d.Interpret EKG's, labs, and radiology/test results and identify findings requiring interventions or follow-up.
e.Monitor patients on an ongoing basis, evaluate plan of care and health outcomes to adjust intervention to ensure optimal delivery of care.
f.Prepare discharge paperwork, arrange for appropriate referral and follow-up after discharge.
g.Sign out to Cardiology fellow who covers MOS service overnight and on weekends.
h.Consult with collaborating physician regarding unusual or complex cases.
i.Collaborate with supervising physician and other members of the multidisciplinary team to ensure optimal delivery of care to patients.
j.Perform discharge teaching and provide prescriptions for same day procedures.
k. Assess and triage patient concerns and symptoms over the phone, including prescription refills as needed.
I.Perform wound checks and device interrogation for patients with urgent needs.
m.Collaborate EP scheduling with Referral Center
